wellness card faq
Any individual living on Cape Cod or the Islands
AND who has been diagnosed with cancer in the past 12 months
OR who has received positive genetic testing results for inherited cancer risk and is undergoing prophylactic surgery
is eligible to apply for the Wellness Card program.
The Wellness Card is a gift from Cape Wellness Collaborative that offers access to integrative therapies such as massage, acupuncture, Reiki, yoga, and more. The card functions like a gift card and is accepted by a network of affiliated practitioners across the Cape and Islands.
Effective August 27, 2024, all clients receive a one-time gift of $750 to use with affiliated practitioners.
Your Wellness Card is valid for one year from the date of your approval letter. Your expiration date is listed in your welcome materials and must be adhered to. Unused funds cannot be reactivated after the expiration date.
Unspent funds represent financial liabilities in our bookkeeping and reporting, and impact our ability to serve new clients.
Key reasons for the one-year expiration:
We must account for all gifts in our financial reporting
Unspent funds affect our annual budgeting and liability
Clear expiration dates help ensure fairness and consistency
Extending cards indefinitely would limit our ability to serve new clients in need
Unfortunately, we are not able to extend expiration dates. While we understand that life during and after cancer treatment can be unpredictable, we must remain consistent with our policies to be fair to all clients and to ensure responsible stewardship of our limited resources.
No. The Wellness Card is non-transferable and may only be used by the client to whom it was issued.
Unfortunately, due to the growing demand for our services, we are only able to support one (family member or friend) caregiver per client. Each approved caregiver receives a $200 Wellness Card to use with any of our affiliated practitioners.
If your services exceed the $750 gift, you are responsible for the overage. Neither Cape Wellness Collaborative nor our affiliated practitioners are able to cover additional fees. Be sure to clarify fees in advance with your practitioner.

1. Visit our Practitioner Directory to choose from our affiliated providers.
2. Schedule an appointment directly with the practitioner.
3. Present your card at the time of service.
4. The practitioner will submit the bill to CWC (up to the value remaining on your card).
Once your $750 has been fully used or the expiration date passes, your card becomes inactive. You’re still welcome to stay connected with Cape Wellness Collaborative and participate in other programs we may offer.
